13项心理健康工具 mwTool-13 可识别四类可能的精神疾病

Brief Rating Tool Identifies Four Categories of Probable Mental Illness

Why It’s Relevant

Most existing screening tools assess the potential presence of one disorder. A single, brief, and easy-to-use mental health tool that can screen for a variety of common psychiatric conditions could be valuable in busy settings such as primary care clinics or emergency departments.

MwTool-13 is designed to screen four categories in 10 minutes or less: common mental disorders (depression, anxiety, or PTSD), severe mental disorders (psychosis or mania), substance use disorders, and suicide risk.

By the Numbers

  • MwTool-13 was administered to 269 patients receiving care in a psychiatric emergency department at an academic medical center in New York and compared with routine psychiatric evaluation.
  • Using standard evaluation, 49% of the participants were given a diagnosis of a common mental disorder, 44% were given a diagnosis of a severe mental disorder, 23% were given a diagnosis of a substance use disorder, and 22% were identified as having an elevated risk for suicide.
  • MwTool-13 was highly sensitive overall, correctly identifying 95% of patients with a diagnosed condition. Sensitivity was 97% for common disorders, 65% for severe disorders, 80% for substance use disorders, and 93% for suicide risk.
  • Specificity (accurately identifying patients who did not have a disorder) was lower: 67% for common disorders, 71% for severe disorders, 58% for substance use disorders, and 50% for suicide risk.

The Other Side

The study relied on routine psychiatric evaluation as the standard, as opposed to using established rating instruments. Additionally, important differences may exist in the type and acuity of psychiatric conditions encountered in psychiatric emergency departments compared with community settings.

Takeaway Message

Pairing the mwTool-13 with brief, evidence-based interventions could facilitate a stratified approach to care that maximizes resources, personalizes care, and reduces waitlists for specialized services. More research is needed to determine how this tool can improve access to mental health care for underserved minoritized communities.
